How to submit
​If you can scan in your completed application form, follow the Electronic Submission Process.  Otherwise, follow the Postal Submission Process

Electronic Submission Process

1. Go to the Go Smart portal.
2. Log in.
   If you have not logged on before, create a new account by clicking here.
3. ​Go to Requests.
4. Create a new request and attach the scanned application form.

Postal Submission Process
Include your completed application form in an envelope and post to:
